Welcome to your Flexible Benefit Plan

Your Employer has asked Total Benefit Services to help them administer your Flexible Benefit Plan. The following information explains the reimbursement process and answers some frequently asked questions:

• On payday your Employer deposits your Flexible Benefit dollars into a separate checking account and sends TBS payroll information so that we can credit your account with the proper amount.

• In order to receive reimbursement for Dependent Care and Medical Related Expenses you must complete a reimbursement request form, attach a copy of a bill or receipt, and send it to the following address:

• The bill or receipt should show the date the service or expense was incurred, the type of service and the amount that you are responsible for paying.

• The date that the expense was incurred (the date of the visit tothe doctor's office, the date the child was in day care) is relevant and not the date of the invoice or the date of your  payment. IRS regulations state that the expenses must be incurred during the period of coverage to qualify for reimbursement.

• If there is not enough money in your Dependent Care account to pay the full amount of the claim, we will reimburse what is in your account and hold the balance of the claim until new  money is added to your account on the next payday.

• For eligible Medical Related Expenses current regulations allow us to pay you up to your total commitment for the plan year.

• Reimbursement requests are processed weekly.

• We are unable to process Dependent Care checks for less than $20 unless it is at the end of the plan year to clear a balance.

• You will be given periodic statements so that you are aware of your account balance.
